Birth defects of the skin are very common. Some of them are only cosmetically serious, others can be part of other organs or gradually turn into malignant tumours. Proper treatment of congenital skin defects is therefore important in terms of quality of life of patients and their families, as well as with regard to the possible medical severity. The most common congenital skin defects are congenital pigment nevi (birthmarks) and vascular disorders (hemangiomas).
Congenital pigment nevi are usually present at birth in 1% of newborns. The most serious are the so-called giant congenital nevi. These are larger than 20 cm in diameter and can cover large areas of the body. They occur in one in 20,000 newborns, i.e. there are about 500 patients with this birth defect in the Czech Republic. Although it is a rare disease, it can be life-threatening (up to 5% may develop malignant melanoma as a complication), have symptoms of central nervous system involvement, and significantly affect the quality of life of the patient and his/her family.
Hemangiomas are the most common congenital skin defect, present at birth in up to 2.5% of newborns and affecting 5-10% of children within one year. After a transitional growth phase, spontaneous healing occurs in most cases. However, children can have a number of complications, which include the development of a surface ulcer, bleeding, infection and very rarely, cardiac complications. Depending on where the hemangioma is located, the patient may have visual, respiratory or internal organ involvement.
A proper and early diagnosis of the congenital skin defect and recommendation of the most appropriate treatment is necessary for each patient. That is why the Consultancy for birthmarks and congenital skin defects has been newly opened at the Bulovka University Hospital. The advisory works as a team of specialists who have been dealing with the problem of congenital defects for a long time
During a consultation with the team of experts, the parents of patients or the patients themselves learn how to deal with their specific defect, when to plan the correct surgical procedure, and how the procedure itself proceeds. Plastic surgery is performed for major congenital defects. In addition, treatment with lasers (vascular and ablative) is possible for hemangiomas. At the same time, a follow-up plan is determined at the respective departments.
If the patient is booked for an outpatient surgery (under local anaesthesia), the surgery is performed at the Plastic Surgery Clinic without hospitalisation. The surgical procedures performed by the plastic surgery team are excision and direct suture or local flap shift, serial excisions, expanders, excision and skin grafting.
If the patient (child under 18 years of age) is booked for surgery under general anaesthesia, he/she is admitted to the Department of Paediatric Surgery. Standard and superior rooms are available here. The stay of a mother with a child up to 6 years of age is covered by the health insurance. Other options for a mother and child (i.e. mother and child over 6 years of age, stay in a superior room) are not covered by the health insurance and are therefore chargeable. Price list for 1 night stay: standard room (mother + child over 6 years old) 500,-CZK, superior room (mother + child under 6 years old) 880,-CZK and superior room (mother + child over 6 years old) 1880,-CZK.
If an adult patient is booked for surgery under general anaesthesia, he/she is hospitalized at the Plastic Surgery Clinic or Dermatovenerology Clinic. In this clinic, patients are usually examined and further monitored using the most modern diagnostic and treatment methods - digital dermatoscopy, confocal microscopy, histopathological examination of skin samples, photodynamic treatment, high-power laser treatment, etc. Patients are also offered cosmetic consultation including aesthetic medicine procedures (medical make-up, scar care, etc.).

Diagnosis and treatment of breast cancer
The specificity of the treatment of breast cancer and all breast diseases lies in a relatively broad multidisciplinary, or interdisciplinary, collaboration. The success rate of treatment is higher in large specialised centres. In the Czech Republic, these are the so-called "Comprehensive Cancer Centres".
The diagnosis and treatment of breast disease involves the cooperation of a radiologist, an oncologist, a nuclear medicine specialist, a pathologist, a gynaecologist, a surgeon, a plastic surgeon and, of course, the next and most important member - the patient.
One of the main programs of the work of the Clinic of Plastic Surgery of Bulovka University Hospital is mammological issues and the clinic is the only workplace in the Czech Republic, which has the status of "Centre of comprehensive care for women with breast disease". The Comprehensive Mammology Care Centre is included in the Comprehensive Oncology Centre (COC) of the Bulovka University Hospital.
Our centre offers patients a full spectrum of breast procedures, from breast cancer surgery to breast reconstruction after ablation, including the most complex microsurgical procedures. Our approach is specific in that, while maintaining oncological radicality, we also emphasize the cosmetic outcome of these surgeries from a plastic surgeon's point of view.
We perform removal of benign lesions, breast salvage procedures, breast removal with skin sparing, or immediate breast reconstruction with an implant or your own tissue. We also perform procedures in the armpit area on the nodes - removal of the sentinel node, or removal of all nodes from the armpit. Surgical care is performed in conjunction with the other aforementioned specialties in the comfort and facilities of the Bulovka University Hospital.
About breast cancer
Breast cancer is the most common malignant tumour in women. The risk of developing breast cancer in an unrisked woman is 8-10%.
Because of the interconnectedness of a woman's hormonal status and changes in the mammary gland, there is an obvious link to female sex hormones, primarily estrogens. Increased exposure to estrogens, cystic adenomas or ductal papillomas in the breast, ionizing radiation, and obesity (increased production of estrogens in fat cells in obesity) all contribute to the increased incidence of breast cancer.
The next chapter is, of course, the hereditary (genetically determined) form of breast cancer, most often in patients with proven BRCA1 or BRCA2 gene mutations. The common denominator is a high risk of breast cancer. The only possible prevention of breast cancer is removal of the affected organ, i.e. removal of the breast gland. Onco-surgical procedures on the breast are followed by reconstructive procedures using breast prostheses - implants or own tissue.
