Centre for Tobacco Dependence
Chief Medical Officer:MUDr. Karolina Rúcklová
- Tobacco dependence brief interventions take place in all departments of our clinic, but the Centre for Tobacco Dependence is dedicated to the highest form of treatment, i.e. a comprehensive tobacco cessation strategy. This involves a combination of psychotherapy training for risk situations and usually medication support for cessation. We monitor the progress of cessation by questionnaire tests and also by measuring tobacco fumes in exhaled air.
- The initial examination at our centre takes approximately 45-55 minutes, with follow-ups taking about 15-20 minutes.
- The success rate of our treatment is around 40-45% in the long term, with a maximum quit rate of up to 5% without medical assistance.

The laboratory provides diagnosis of sleep-disordered breathing and their treatment.
Nightly sleep monitoring includes:
- Polygraphic monitoring with Porti 7 device which monitors breathing, blood oxygen saturation, snoring, chest and abdominal breathing effort, ECG curve, transit pulse time, body position during sleep. We have 3 of these devices. Another device of the same category, the NOX T3, complements the above devices and senses the same parameters in addition to the ECG curve.
- Polysomnographic monitoring Porty 8 monitors four EEG electrodes, two eyeball movements and two chin muscle tension electrodes in addition to polygraphic parameters. The monitoring is an online recording of the image and sound, which is captured overnight by a camera and microphone.
- Treatment of all forms of obstructive sleep apnoea and hypopnoea by regimen measures, surgical plastics at the ENT clinic of the Bulovka Hospital or treatment with CPAP and VPAP non-invasive ventilatory support in monitoring room with bed. The monitoring is usually done 4 days a week.
The laboratory staff consists of a full-time physician, two part-time physicians and one part-time laboratory technician. The laboratory has two monitoring rooms with beds. Monitoring is usually done every weekday. Polysomnographic monitoring occurs once a week, always on Thursdays.

- Examination of diffusing capacity of the lungs.
- Exercise spiroergometry - exercise, expiratory.

Home non-invasive ventilation
Lead Physician: odb. as. MUDr. Jaroslav Lněnička
We treat patients with chronic hypercapnic respiratory insufficiency including non-invasive ventilatory support in neuromuscular diseases and "High-intensity NIV" in COPD. Furthermore, we offer the indication and setting of airway depressurization therapy (oxygen therapy) for home non-invasive ventilation in patients with complicated sleep-disordered breathing (overlapping COPD + OSA, OSA + hypoventilation syndrome in obesity, OSA with hypoxia in comorbidities, etc.) or in patients in whom the sleep laboratory has failed to satisfactorily set up pressor therapy.
Patients are examined and introduced to treatment either on an outpatient basis or during hospitalization depending on the patient's ability to commute, and primarily on the severity of the disease.
An appointment can be made by calling 266 08 2081 or better yet, by email atjaroslav.lnenicka@bulovka.cz.
The referring physician's referral and detailed medical records are required for the examination. For patients already examined in the sleep lab, we also ask for electronic data from the polygraph and outpatient titration so that we can smoothly follow-up and not repeat tests already performed.

The department is focused on the diagnosis of lung diseases by bronchoscopic examination, which is performed mainly under local anesthesia with a flexible bronchoscope, exceptionally under general anesthesia with a rigid bronchoscope. Bronchoscopic examination is used to diagnose diseases of the airways and lung parenchyma by means of the brush method, endobronchial aspiration and excision. We also perform bronchoalveolar lavage and perbronchial puncture. Bronchoscopic airway toileting is performed in long-lying patients, and inhaled foreign bodies are removed by bronchoscopy.
