E-mail. Please fill out this onlineform download it (here) and email the completed form to the email above. You will need the following (if available) to complete it:
- CT scans (ePACSem),
- MR scans (ePACSem),
- epicrisis,
- anamnesis,
- performance status,
- basic lab results (including oncomarkers),
- results of related tests performed to date (ultrasound, CBC, GFS, etc.),
- contact of referring physician,
- contact and nationality of the patient (name, birth number, insurance company, address, telephone, weight, height).
