Mamodiagnostic Centre

The Mammocentre is part of the Radiodiagnostic Clinic of the Bulovka University Hospital. Our workplace is one of the centres accredited by the Ministry of Health for performing mammography screening - a nationwide health programme in the Czech Republic, the main objective of which is early detection of breast cancer by imaging methods.

We perform diagnostic and preventive mammography, 3D mammography - tomosynthesis examination, contrast mammography, ultrasonographic examination (sono, ultrasound, ultrasound) of breasts, ductography, thin-needle and vacuum breast biopsy, breast magnetic resonance imaging.

The principle of mammographic screening is to perform clear mammographic images in asymptomatic women, two projections per side. Evaluation of mammographic images is performed independently by two physicians, which increases the sensitivity (sensitivity) of the mammographic screening. The examination is performed on a Hologic digital mammography machine with and Siemens Inspiration.

Mammographic breast examinations

Mammography/mammographic screening is the basic screening method for women over 40 years of age. 

Screening Mammography - an examination covered by health insurance

Any woman who has no symptoms of breast cancer and:

  • is over 45 years of age (no upper age limit),
  • has a referral for screening mammography from a gynaecologist or general practitioner.

The examination is repeated, at two-year intervals (since the previous mammogram) and is covered by health insurance.

Screening Mammography - Self-Pay

Screening is performed in women who are not experiencing symptoms of breast cancer, and at the same time:

  • are over 40 years of age,
  • at least 1 year has passed since their previous mammogram.

A request for screening is not required in this case. An example is women taking hormone replacement, who are generally recommended to have a preventive breast examination every year.

Diagnostic Mammography

Examination of women with previously diagnosed breast cancer, or women with complaints that may be symptoms of breast cancer. Such symptoms include, but are not limited to, a palpable lump, skin retraction, nipple retraction, unilateral nipple discharge, inflammatory breast disease, etc. A request from the doctor who requires the examination is a prerequisite.

Diagnostic Mammography - Self-Pay

It is indicated for women before breast reconstruction for cosmetic reasons (reduction, modelling, implants), without age limitation.

Additional Mammography

Special projections to complement standard mammography images to rule out pathology, summarize glandular structures, or clarify unclear findings.

Ductography

 

Examination performed when there is a pathologic secretion from the nipple. The principle is mammographic imaging of the mammary ducts (ducts) in two perpendicular projections, using an iodine contrast agent.

Ultrasound breast examination

The examination is performed on two Toshiba Aplio ultrasound machines, equipped with a special linear high-frequency breast probe for detailed imaging of breast tissue and lymph nodes in the armpit.

Preventive Breast Ultrasound

  • self-pay women,
  • age is not limited, it is performed mainly in women under 40 years of age where it is the first choice method, and also in women dispensed for BRCA1 and BRCA2 gene mutations.

Ultrasonography as an adjunctive examination to screening mammography

  • based on the recommendation of the radiologist who evaluated the screening mammography, usually performed immediately,
  • no later than 6 months after screening mammography on the recommendation of one of theof the radiologist who evaluated the mammogram - in this case it is covered by the health insurance, then for direct payment for the additional examination if the recommended date is met, there is no need for a new request.

Diagnostic Breast Ultrasonography

The same conditions apply here as for diagnostic mammography, a referral from the referring physician is always required.

Breast MRI

The examination is performed in the prone position, in selected medical indications and usually preceded by other imaging methods.

  • examination with intravenous application of gadolinium contrast agent to detail the pathological breast lesions based on their saturation with contrast agent,
  • examination without contrast agent to detail the condition of breast implants.

Breast biopsy

Extraction of material for histological examination (with a thin needle or vacuum) to exclude or confirm the presence of cancer and determine its type.

  • Ambulatory examination, always performed under local anaesthesia.
  • The target of the biopsy can be detected under sonographic, mammographic and magnetic resonance imaging control.

Preventive screening of women at high and intermediate risk of breast cancer

Women under 45 years of age are also entitled to preventive examinations covered by health insurance and performed at intervals according to specific standardized schemes:

  • women whose risk of breast cancer is determined by genetic testing to be 20 % or more (carriers of gene mutations, women with high familial risk determined by genetic testing - so-called. high risk),
  • who have undergone chest radiotherapy at a young age.

Women with risk of breast cancer of less than 20 % (called intermediate risk) are covered by health insurance for mammograms from the age of 45 at two-year intervals, other recommended preventive examinations are paid for by the women themselves.
