Information on the cause of death is given:

Oral (personal meeting with the doctor)

A meeting with the doctor who performed the autopsy is possible by appointment. To use this option, please contact the secretariat of the institute at 705 504 964  MON-FRI from 13.00 to 14.15, where you will receive a telephone contact to a specific doctor with whom you can arrange an appointment. 

In writing

To have a written copy of the post-mortem report sent to you (or to collect it in person), you must complete the following forms: 

1. Application for a copy of the post-mortem findings  
2. Affidavit by the applicant for a copy of the post-mortem report, certifying the family relationship (alternatively, a copy of the marriage certificate or birth certificate may be provided)
3. We also require a photocopy of both sides of the applicant’s identity card (if you do not wish to send a photocopy of your identity card, you must submit an ‘Application for an Autopsy Report’ bearing an officially certified signature) 

Please send all supporting documents either by post to the address below or by email to soudni@bulovka.cz (scanned in ‘PDF’ format). Once completed, the report will be sent cash on delivery to the address you specify in the application, subject to a fee as per the current price list for services not covered by health insurance  plus a postage fee in accordance with the Czech Post’s tariff (all payable upon collection at the post office). You may also collect the report in person from the institute’s secretariat, where you will only be required to pay the fee for issuing a copy of the post-mortem report in accordance with the current price list. If collecting in person, please arrange an appointment in advance with the secretariat; the copy can be collected Monday to Friday from 1.00 pm to 2.15 pm. The report can also be sent to your data box, subject to prior payment of the fee for issuing the copy via an invoice. A written copy of the post-mortem report is intended solely for your personal use, and you are not obliged to pass it on to anyone else! The doctor has one month from the date of receipt of the request to prepare the copy of the post-mortem report. 

As soon as the report is ready, we will inform the applicant immediately.

Application by the deceased's GP

The practitioner will send the completed and stamped application form either by post or by e-mail to soudni@bulovka.cz. A copy of the autopsy report will be sent to him/her after completion.

Postup pro vyzvednutí cenností

Cennosti předá dědici/dědicům pověřený pracovník ÚSL na základě předloženého pravomocného rozhodnutí soudu o nabytí dědictví a průkazu totožnosti. V případě, že se dědic nemůže dostavit osobně, jeho zástupce je povinen předložit plnou moc k převzetí cenností, podpis dědice na plné moci musí být úředně ověřený. Cennosti lze vyzvednout v úředních hodinách v pondělí od 12.30–14.30 a středu 12.30–14.30. pouze po předchozí domluvě e-mailem na adrese pavz@bulovka.cz nebo telefonicky na čísle 26608 3468 nebo 604 280 090. Cennosti převezme dědic v ÚSL, budova č. 17 od pověřené osoby.
